When registering with Grange Family Practice you will be registering with the Practice and not an individual GP.
Persons applying for National Health Service registration for the first time please use the HSCR1 form and provide the supporting documentation as requested on the registration form and a recent utility bill and photographic I.D. Please see attached HSCR1
In order to register anyone who is not a UK or Irish National, entering the UK from 1st January 2021 for the first time, they will be also required to provide:
Their passport or EU ID card, and one of the following:
- a Biometric Resident Permit (BRP) or a copy of your share code printout
- proof of being a qualified person, or
- satisfy a visitor exemption listed in The Provision of Health Services to Persons Not Ordinarily Resident (Amendment) Regulations (Northern Ireland) 2015.
These changes do not apply to EU Nationals who entered the UK pre 01.01.21 and have been granted ‘pre settled’ or ‘settled status’.
Alternatively if you are currently registered within the NHS or have been within the last 12 months please use the HS200 form. The practice also requires a recent utility bill (proof of address), and photographic I.D. Please see attached HS200
All persons wishing to register are also asked to complete a new patient health summary. Please see attached New Patient Questionnaire and for children under 6 please please ensure you complete the vaccination history section of the questionnaire. Registrations for children under the age of 6 years will not be accepted without a copy of Childhood Immunisations/Vaccinations.
Your registration form will be forwarded to Business Services Organisation who will issue you with a new Medical Card. This takes approximately 6 weeks.
If you move out of the practice area, you will need to register with a new practice as soon as you move. If you have difficulty in getting registered in a new practice you should contact:
