New Patient Registration

To apply to become registered with our practice, you should contact the practice and leave the following details -Name, Date of Birth, Full Address including Postcode, Previous GP or if new to N.I - Address from outside the U.K. The practice will review your details and if you are within the 7 mile catchment area of Omagh Hospital and Primary Care Complex, the appropriate registration form and Health Questionnaire will be posted to you to complete, please note the details on the cover letter.

When you register with Grange Family practice you will be registering with the practice, not an individual GP. You can however request to see a particular GP at any time or different GP's for a particular case.

Change of Personal Details

If you change your address etc. please notify reception staff as soon as possible, so that our records are kept up-to-date.

Temporary Registration

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.

To be registered as a temporary patient you must have an address which is within the 7 mile catchment area and have a family member / relative registered with the practice.
